Care coordination is a collaborative effort to provide treatment resources to clients that manages and reduces personal risk, and promotes accurate diagnosis and treatment through the process of effective information sharing. This collaboration connects behavioral health, primary care, peer and natural support, housing, education, vocation, and justice systems to assist individuals in transitioning from higher levels of care to community-based services. By linking different service providers, Care Coordination can enable the sharing of information, employ joint planning efforts, and foster collaborative treatment options.
The coordination of services incorporates shared decision making in all phases of care and emphasizes self-management to allow individuals to be the driver of their own personal goals. Care Coordination assess for and addresses behavioral health issues as well as medical, social, housing, and interpersonal problems/needs that impact the individual and their family support network.
